Planning Officer Job Description Template
Our company is looking for a Planning Officer to join our team.
Responsibilities:
- Conduct assessments utilising TPS approved tools;
- Work within a multidisciplinary team and participate in case-management reviews to ensure continuity of care;
- Case Co-ordination of a varying number of clients as required;
- Provide evidence based criminogenic assessment for offenders identified as suitable for a coordinated intervention;
- Develop comprehensive case plans and make referrals and or recommendations in line with individual case plans;
- Participate in supervision, peer review and continuing professional development and risk management practices;
- Convene and lead case conferences, liaise with other prison staff and relevant agencies about assessment and case management;
- Discuss with inmates the results of assessment and formulate sentence goals.
Requirements:
- Degree in Social Work, Psychology or Counselling.
