Our company is looking for a Social Worker to join our team.
Responsibilities:
- Complete relevant paperwork, case notes and data entry;
- Conduct intake and assessment to determine needs;
- Provide crisis support to clients with high risk and complex needs;
- Advocate for clients and liaise with a range of external stakeholders;
- Develop and review case plans to achieve mutually determined goals.
Requirements:
- You are passionate about providing evidence-based practice in collaboration with clients and their families/carers;
- Degree in Social Work giving eligibility for membership of the Australian Association of Social Workers; or;
- Current drivers licence;
- A minimum of two years post graduate employment in Social Work in clinical area applicable to Alcohol and Drug Services.
