Case Worker Job Description Template
Our company is looking for a Case Worker to join our team.
Responsibilities:
- Work with clients to create individualised support plans including referral to supplementary services as needed;
- Respond to referrals of clients to the service and conduct over-the-phone and formal face-to-face assessments of suitability for support;
- Provide intensive ongoing case management sessions with clients and review progression against case plans and provide informal counselling as required;
- Induct clients into the service including the property, facilities, financials and regulations;
- Undertake initial registrations for clients, including all necessary paperwork and application forms.
Requirements:
- You will possess complex mental health experience;
- A Degree or Diploma in Social Welfare/Social Science or substantial relevant experience in a similar role;
- One on one counselling and group facilitation;
- Drives Licence;
- Outreach and centre based work with clients;
- Experience in working within a Recovery framework.
