Younger Onset Dementia Club Facilitator Job Description Template
Our company is looking for a Younger Onset Dementia Club Facilitator to join our team.
Responsibilities:
- In conjunction with the manager monitor and review consumers’ needs and progress;
- Assists with appropriate delegation and support for work colleagues and volunteers;
- Facilitate purposeful and individual stimulating outings and activities of the consumers choosing;
- Assist consumers with the development, planning and co-ordination of new and current programs;
- Share the responsibility of general hygiene, cleanliness and tidiness of the Young Onset Club with consumers;
- Undertake personal care duties for individual consumers when necessary;
- Preparing, opening and closing the facility;
- Complete progress notes and profiles on consumers;
- Co-ordinating, training and engaging volunteers.
Requirements:
- Able to drive a mini bus;
- Drivers Licence;
- Current Police Check;
- Certificate 3 or IV in Aged Care;
- First-Aid.
