Our company is looking for a Specialist Support Coordinator to join our team.
Responsibilities:
- Provide advice and consultation by making phone calls to participants, families and external service providers;
- Prepare comprehensive reports in conjunction with relevant stakeholders;
- Liaising with colleagues and discussing service options for participants;
- Perform Specialist Support Coordination to assist participants in implementing their NDIS plan;
- Coordinate services in participants’ NDIS plans including the management of complex multi- disciplinary teams.
Requirements:
- Demonstrated ability to develop positive relationships with a range of participants, their families, and their service providers;
- Tertiary qualification and/or significant experience working in the Community Services sector (Disability, Mental Health, Housing, Justice);
- NDIS awareness, particularly in relation to support coordination and the importance of choice and control for participants;
- Social Work/ DE or other allied health (disability experience).
