Our company is looking for a Specialist Support Coordinator to join our team.

Responsibilities:

  • Provide advice and consultation by making phone calls to participants, families and external service providers;
  • Prepare comprehensive reports in conjunction with relevant stakeholders;
  • Liaising with colleagues and discussing service options for participants;
  • Perform Specialist Support Coordination to assist participants in implementing their NDIS plan;
  • Coordinate services in participants’ NDIS plans including the management of complex multi- disciplinary teams.

Requirements:

  • Demonstrated ability to develop positive relationships with a range of participants, their families, and their service providers;
  • Tertiary qualification and/or significant experience working in the Community Services sector (Disability, Mental Health, Housing, Justice);
  • NDIS awareness, particularly in relation to support coordination and the importance of choice and control for participants;
  • Social Work/ DE or other allied health (disability experience).
