Care Manager

About the Role:

We are seeking an experienced Registered Nurse to join our community-based team as a Care Manager.

In this rewarding role, you will support clients to live safely and independently in their own homes. You will assess client needs, develop and review individual care plans and coordinate high-quality community support services.

You will also provide clinical supervision, coaching and ongoing support to Support Workers working throughout the local community.

Key Responsibilities:

• Act as an expert clinical resource
• Assessing clients in their homes using interRAI and other computer-based tools
• Providing clinical guidance, training and supervision to Support Workers
• Providing assessment in collaboration with family/whanau and other primary health providers.
• Developing individual plans in collaboration with the client, family/whanau
• Coordinating clinical requirements, including clients' reviews and competencies
• Maintaining key relationships with stakeholders, clients, families/ whanau, and other agencies.
• Delivering services aligned with contractual obligations

Skills and Experience:

• Current NZ Registered Nurse registration and Annual Practising Certificate (APC)
• Experience in community care, or a strong understanding of community care
• Knowledge of working with older people and/or those with disabilities
• Strong leadership, mentoring and interpersonal skills
• Valid full NZ driver's licence
