Job Description
We are seeking a compassionate and experienced Registered Nurse to join our Homecare team, supporting the delivery of high-quality clinical care to clients living independently in the community.
Reporting to the Customer Care Manager , you will play a key role in providing clinical coordination, case management, and nursing support to Homecare Package clients. This role is ideal for someone who enjoys a mix of clinical work, care planning, and stakeholder engagement.
Key Responsibilities
- Deliver high-quality, person-centred nursing care to clients in their homes
- Conduct assessments, develop and review care plans to meet changing client needs
- Provide clinical guidance and respond to changes in client health status
- Maintain accurate and compliant clinical documentation
- Collaborate with multidisciplinary teams to ensure optimal care outcomes
- Build strong relationships with clients, carers, GPs, and community stakeholders
- Support continuous improvement initiatives and uphold best practice standards
- Ensure compliance with Aged Care Quality Standards, WHS, and organisational policies
- Participate in mandatory training and ongoing professional development
About You
You are a dedicated and proactive Registered Nurse who is passionate about supporting older people and individuals living with disability to live independently.
- Strong clinical knowledge and sound judgement
- Excellent communication and relationship-building skills
- The ability to work autonomously and within a team
- A genuine commitment to quality care and continuous improvement
Key Requirements
- Bachelor of Nursing with current, unrestricted AHPRA registration
- Minimum 5 years’ post-graduate experience (aged care, community, or hospital setting)
- Experience in wound care, catheter care, assessments, and care planning
- Solid understanding of aged care standards, WHS, and risk management
- Current First Aid Certificate and National Police Check
- Valid driver’s licence and access to a reliable vehicle
