NDIS Community Access Coordinator

We are seeking a proactive and compassionate NDIS Community Access Coordinator to join our Disability Services team.

About the role

As the Community Access Coordinator, you’ll play a vital role in connecting participants with JewishCare’s NDIS services. This position is ideal for someone who thrives in a dynamic environment and is passionate about inclusive community engagement.

Your key responsibilities will include:

• Assisting participants to understand and access NDIS supports
• Light touch case management and NDIS plan implementation
• Supervising and supporting NDIS Support Workers to ensure high-quality service delivery
• Coordinating rosters and ensuring compliance with documentation and funding requirements
• Engaging clients in meaningful community activities and opportunities

What you’ll need to succeed

• Relevant qualifications in Social Work, Allied Health, Community Services or similar
• Sound knowledge of the NDIS, quality standards, restrictive practices and incident management
• Strong communication, time management and problem-solving skills
• Experience supporting individuals with intellectual and/or developmental disabilities
• Ability to supervise teams and manage client documentation systems (e.g. GoldCare)
• Current Driver’s Licence and willingness to conduct home visits

Desirable

• Familiarity with Jewish values and community is advantageous
